Organizational Participation Consent

Informed Consent for Workplace & Corporate Wellness Services

This consent applies to employees, managers, leaders, and other participants in organization-sponsored wellness, communication, leadership, training, or facilitated programs.

Nature and Scope

Services are emotional wellness coaching, education, guidance, and skills support—not psychotherapy, medical care, psychiatric treatment, diagnosis, or emergency services.
The service does not replace care from a licensed medical or mental-health professional.
Mary may recommend appropriate outside professional support when a need falls beyond her stated scope.

Participation and Responsibilities

Participation does not create an employment, medical, therapy, or legal relationship with Mary.
Mary does not make employment decisions or provide legal, human-resources, medical, or psychological evaluations.
Unless separately agreed in writing, Mary provides the sponsoring organization only with program-level information—not private personal disclosures.

Privacy and Its Limits

Participants should not share confidential employer, customer, patient, trade-secret, or personnel information. Any reporting to the sponsoring organization will follow the written service agreement and applicable disclosure notices.

Information may be disclosed when permission is provided, when required by law or valid legal process, or when action is reasonably necessary to address an immediate safety concern. Do not submit sensitive wellness details through ordinary email, text messages, or public website fields.

Crisis and Safety

Mary’s services are not crisis or emergency services. Call 911 for immediate danger or call or text 988 for a mental-health crisis. Do not wait for a website, email, text, appointment, or coaching response.

Acknowledgment

By continuing in the secure client-start journey and providing your electronic signature, you confirm that you had the opportunity to review this category-specific consent, understand the nature and limits of the service, may ask questions before participating, and voluntarily agree to proceed.

This consent works together with the applicable service agreement, Scope of Practice, Privacy Policy, cancellation/payment terms, and—when Zoom is selected—the Telehealth Consent.
