Virtual sessions are conducted through Zoom instead of an in-person visit. Mary sends the private Zoom meeting link only after the appointment is confirmed. Before participating, confirm that you can join from a private location and ask Mary any questions you have about privacy or virtual-session procedures.

• ▪You will join sessions from a private, quiet location free of distractions.
• ▪You are responsible for a stable internet connection and a working device with camera/microphone.
• ▪You understand that if you are in crisis, telehealth is not appropriate — contact 911 or 988 immediately.
• ▪You will notify the Practitioner of your physical location at the start of each session in case of emergency.

By continuing with telehealth services, you acknowledge that you have read, understood, and agree to the terms of this Telehealth Consent Form.
